The time to achieve Goal No. 10 in full has arrived. Club Presidents

should have received the Semi-annual report forms in late August. Some of

you may have sent your report form with your bank draft to WHQ. If you have not, these are the steps to complete the semi-annual report form and payment.

Semi-Annual Reports MUST include :

(1) a completed Report Form

(2) Membership list of current members

(3) Bank Draft for total number of members x US$18

(4) All dues must be paid up to and including September 2001.

Please DO NOT include newly recruited members' fees. A separate bank

draft

is required for payment of New Members' fees together with their

application

forms (Form 400).

For NEWLY CHARTERED CLUBS, please TAKE NOTE that you will need to cover

the

outstanding months up to and including September 2001 to cover period not

paid by the initial six months' advance payment. For example :

If your Club was chartered in January 2001 -

Your initial six months' advance per capita payment of US$18 will cover up till June 2001. In this instance, your Club will need to pay the pro-rated amount of US$9 per member (US$3 per month) for the period July 2001 to

Sept 2001.

BY COURIER SERVICE

I have just checked with the courier service and to the best of their knowledge, all their mails to California have been delivered. However, we must take note of the tight security checks which may cause some delay in delivery to World Headquarters.

Another point to remember - Courier services DO NOT deliver to post office boxes. If you are sending your October Semis BY COURIER to World Headquarters, please note the street address as follows :

Your semis should arrive at World Headquarters by October 1. Airmail would normally take 10 days or more. It may take longer in view of the current tight security in the USA. Please send by courier.

BY TELEGRAHIC TRANSFER.

Clubs need to provide the following information in the Telegrahic form :

(1) Club number; District number;

(2) What the remittance is for (eg semi-annual dues - October 2001 - March 2002 for number of members)

(3) Thereafter, fax and mail the Telegraghic form, the wire receipt plus the membership list to WHQ. WHQ fax number is : (949) 858-1207

If your Club HAS NOT received your semi-annual report form, please download a copy of the form from WHQ website. Alternatively, please refer to your Area Governor for photocopies of such forms. (Area Leadership Handbook

page 39).

For membership list, use previous list, update the list and send in the membership fees for the total number of current members.
